The callback form collects your name, phone number, and a preferred time to call. The feedback form collects your message and, if you choose, your name and phone number. We ask that you do not send medical information through either form.

How it is used

We use this information only to call you back or to read your feedback. We do not sell it or share it for marketing.

Analytics

The site may count visits and clicks on buttons such as Call to Schedule, so we know which pages are useful. These counts do not identify you.

Maps and reviews

The map on this site is provided by Google. Review links open Google or Yelp, which have their own privacy policies.

Your medical records

Records from your visits are kept at the clinic and handled under federal health privacy law. They are not stored on this website. You will receive the clinic's Notice of Privacy Practices at your first visit.
